Scotts Hill Baptist Church - Youth Ministry Event

CONSENT TO PARTICIPATE: I, the undersigned parent or legal guardian, consent to my child's participation in this Scotts Hill Baptist Church youth ministry event. I acknowledge the inherent risks and grant permission for full involvement in all event activities.

ACKNOWLEDGMENT OF RISK: I understand that participation may involve physical activity, travel, and unforeseen hazards. I affirm my child is physically able to participate and that I have disclosed all medical conditions.

MEDICAL AUTHORIZATION: I authorize Scotts Hill staff and volunteers to obtain emergency medical treatment for my child if necessary and accept financial responsibility for such care.

LIABILITY RELEASE: I release and hold harmless Scotts Hill Baptist Church, its staff, volunteers, and representatives from all liabilities, claims, or demands arising from my child's participation, except in cases of gross negligence or intentional harm.

INDEMNIFICATION: I agree to indemnify and defend Scotts Hill Baptist Church against any claims arising from my child's participation in the event.

MEDIA CONSENT: I understand that photos or video may be taken and used for ministry-related purposes unless I decline permission.
